Karen Trask is a dual-certified Family Nurse Practitioner (FNP) and Psychiatric Mental Health Nurse Practitioner (PMHNP) with more than 28 years of clinical nursing experience. She earned her Bachelor of Science in Nursing from Grand Canyon University in 2004 and completed her Master of Science in Nursing, Family Nurse Practitioner, in 2014. In 2025, Karen obtained her Psychiatric Mental Health Nurse Practitioner certification from Wilkes University, expanding her ability to provide comprehensive, whole-person care.

Karen serves as a preferred provider for several Assisted Living Facilities across the East Valley and brings a broad clinical background that includes Skilled Nursing Facilities, Sub-Acute Care, Urgent Care, and Family Practice.

An Arizona native, Karen is dedicated to supporting the health and well-being of her community’s seniors. She is committed to delivering compassionate, high-quality, patient-centered care with a strong focus on dignity, respect, and improving quality of life.

Cynthia is a board-Certified Family Nurse Practitioner. In 2009 she obtained her Master of Science in Nursing, Adult Nurse Practitioner from Arizona State University. She brings a great deal of knowledge and dedication to her work and has a true heart for those whom she cares for. Outside of work she loves spending time with her son, daughter, and their Saint Bernard Puppy.

Tracey is a board -certified Family Nurse Practitioner. She received her Bachelor of Science in Nursing from Grand Canyon University and worked as a pediatric nurse for 11 years. Deciding to further her career in the field, she received her Master of Science in Nursing from Chamberlain University and transitioned into geriatric care in 2021. Along with her desire to help care for her patients, she brings with her a kind, compassionate and true interest in their well-being. In her downtime, Tracey enjoys traveling with her husband and spending quality time with her children.

Jennifer is a board-certified Adult Nurse Practitioner. She received her Bachelor of Science in Nursing from Purdue University and worked as a cardiac nurse. Seeking warmer weather, she moved to Arizona and obtained her Master of Science in Nursing from Arizona State University in 2004. She enjoys developing relationships with her patients, families and communities so she can deliver knowledgeable and focused care to our geriatric population. In her free time, she enjoys traveling and experiencing new cultures with her children or taking advantage of Arizona's beautiful outdoors.
